negligence

Well, as I said, I have a hard time thinking the server's address book could have been DELETED whiel the server was up and running. I pretty sure the server wouldn't allow the delete to continue.

From your description of what they were doing, perhaps they replaced the design on the names.nsf with some other template, causing this problem.

The description of how they were replacing the mail templates is reasonable. I wouldn't expect it to create a problem. Sound like someone did that when they were clicked on names.nsf, perhaps.
